Primary care is provided to all ages through the family practice clinics in the Richmond and Missouri City center and on board the mobile health clinic. Physicians stress screening for early detection of common diseases and health problems. Services include diagnosis and treatment of acute and chronic illnesses, health education, comprehensive prenatal care, and family planning services. AccessHealth has an agreement with a commercial lab to provide services in the Richmond Center allowing patients easy access to lab services. Lab work samples are picked up by a courier on a daily basis at the other locations.Pharmacy services are offered to all AccessHealth patients. A full-service pharmacy is located at the Richmond Center and limited pharmacy services are available at the Stafford and Missouri City centers and on the mobile health clinic.

The main goal of the Family Practice Clinic is to improve the overall health for patients. The performance improvement program measures and reports health indicators and outcomes for all lifecycles, HIV, geriatrics, diabetes and hypertension. Preventive health measures are aimed at controlling high blood pressure, diabetes, and obesity;increasing the number of patients that have an annual well exam, and improving the immunization rate.
Description
The pediatric specialty clinics in the Richmond, Missouri City and Stafford centers provide primary care for children through 18 years of age. Services include well-child care, immunizations, diagnosis and treatment of acute and chronic illnesses, and education on health related issues.

The Pediatric Clinic closely monitors health outcomes for children, 0 to 18 years. The program monitors immunization and obesity rates, and asthma rates among other outcomes.

Located at the Stafford Center, the dental clinic offers basic preventive, restorative and emergency oral health services, oral hygiene education for patients, and an educational opportunity for dental hygiene students.

The main goal for the Dental Clinic is good oral health. The Dental Clinic focuses on prevention and relief of pain. Encouraging good oral health through regular brushing, flossing, and check-ups is vital to promoting good oral health. Sealants are applied to the teeth in young children for added protection, and mothers are taught how to wipe the gums of infants and clean teeth their baby's teeth.
A specific measure that is monitored is completion of treatment plan. Patients tend to go to the dentist when they experience pain. The Dentist identifies a course of treatment that both relieves the pain short term, and eliminates the oral health issue. Some patients do not return to the dentist for each treatment, and often return with pain months to year(s) later.
Description
AccessHealth provides psychosocial support services through three components: the behavioral health program, the HIV case management program, and short-term assistance for patients with urgent needs.
The behavioral health program utilizes an integrated care model in which primary care and mental health providers partner to manage the treatment of mental health problems in the primary care setting. Clinical care management, including medication management and psychiatric consultation, are elements of the integrated care model. Psychotherapy and crisis intervention are also components of the integrated care management.
HIV medical case managers assist HIV+ patients to manage their disease and the psychosocial issues associated with HIV/AIDS.

The goal for AccessHealth's behavioral health & care management program is to assist patients in maintaining treatment adherence by identifying and removing barriers to achieving medical and mental health treatment goals.
Our behavioral health program uses evidence-based practices and has proven successful, exceeding program outcomes recommended by the Agency for Health Care Improvement. 55% of patients served by the program achieve a 50% or greater reduction in depression scores, exceeding recommended targets established by experts.
Description
WIC provides nutrition and breastfeeding education, health screenings, medical referrals and vouchers for nutritious foods. AccessHealth’s WIC program operates in 12 sites within Austin, Colorado, Fort Bend, Waller and Wharton counties. Services sites are located in the cities of Bellville, Brookshire, Columbus, Eagle Lake, El Campo, Katy, Prairie View, Rosenberg, Sealy, Stafford, Weimer, and Wharton.
WIC clients must meet an income guideline (family income below 185% of poverty), be at-risk nutritionally, and be a pregnant, post-partum or breastfeeding woman, an infant under age one, or a child under age 5.
